Purpose of the Questionnaire:
The questionnaire aims to collect essential information that will allow our panel to make a fair and comprehensive assessment.
Completion Deadline:
Please ensure the questionnaire is fully completed and submitted online by 29th September 2024. Failure to meet this deadline may result in your removal from the MTM Awards Season 12.
