MTM Awards Questionnaire is simple and straightforward, it is important you read the T&C and agree to them before answering the MTM Awards questionnaire.

  • provide the nominee’s name/business/organisation and valid email for verification.
  • answer each questionnaire to the best of your knowledge
  • any material supplied in support of an entry must be your original work. We will not accept an entry that contains work that does not belong to you, has been plagiarised or where we suspect that the entry may fall within the foregoing categories.
  • the entries will be judged by a panel of selected judges.
  • we will consider your entry on its merits and solely on the basis of the information you provided in your entry.
  • we reserve the right to withdraw any particular award in any given category.
  • include evidence of any legacy for organisations or persons in the Special and Community Awards.
  • each question is limited to 500 words
  • the judging panel will take into account information on the questionnaire form only, and links provided
  • ensure you’re answering the right questionnaire in the right category
  • ensure it is the right award.
  • all fields must be filled unless indicated otherwise
  • provide your full name and valid email for verification
  • the decision of the panel of judges will be final and we will not enter into any correspondence or discussion with any entrants nor will we accept any submissions,
  • we reserve the right to withdraw any nominee/finalist/winner and refuse admission to any person whom we consider in our absolute discretion to be unsuitable for an award and remove such person after the start of the awards ceremony. In such circumstances, there will be no refund of the attendance fee.
  • the questionnaire will NOT be accepted after the last date of submission.
  • you give consent to use of your image and or video clip pre and post awards.
  • all international ‘FINALISTS’  will have to provide eveidence that they are able to obtain entry visa to the UK and sustain their travel and upkeep for their trip.
  • all short-listed nominees and finalist are expected to respond to communications from the admin team, failure to do so will affect chances to progress to the next stage.
  • IF YOU ARE A FINALIST FOR AN AWARD YOU ARE REQUESTED TO ATTEND THE EVENT.
